令和7年12月1日をもって、健康保険証が使用できなくなります
令和7年12月2日以降、現在お持ちの健康保険証は使用できなくなります。
使用できなくなった健康保険証については、ご自身で廃棄してください。
令和7年12月2日以降は、マイナンバーカードに健康保険証を利用登録した「マイナ保険証」を提示して医療機関をご受診ください。
なお、「資格確認書」は、引き続きご使用いただけます。
Important Notice Regarding Your Health Insurance Card
To Our Patients,
Your Current Health Insurance Card will no longer be used from Dec 2, 2025.
When you visit our clinic, please present one of the following documents at the reception:
- Your “MyNa” Insurance Card (Your My Number Card that is registered for health insurance use.)
- Your Certificate of Eligibility (Shikaku Kakunin-sho) (This certificate will be issued to those who do not have a My Number Card or have not registered it.)
Our clinic is equipped to handle these options. We recommend using your “MyNa” Insurance Card for the most accurate and efficient service.
